Description/Job SummaryAssertive Community Treatment (ACT) Team is a multi-disciplinary program that provides evidence-based, mobile, community mental health treatment to promote the recovery and community integration of individuals age 18 and up with serious mental illnesses, high rates of co-occurring substance use disorders, and high utilization of emergency/crisis services. ACT Team is a unique program that focuses on providing intensive outpatient treatment to individuals with serious mental illnesses so that they can maintain stability in the community.The Counselor is responsible for assessing and providing treatment to individuals struggling with susbtance abuse symptoms. The role of the Counselor is to support individuals on their journey of recovery by educating them on the skills needed to maintain long-term recovery.Responsibilities/Duties+ Review of clinical information and other relevant documentation about the individual's treatment and life experiences+ Collaborating with clinical team including pyschiatrist, nurse, team lead, case manager, etc. + Providing Individual and Group therapy sessions to help individuals develop healthy, positive coping skills to manage their addiction issues+ Supporting family members and/or significant others in living with an individual who is struggling with addiction issues+ Developing and updating treatment and recovery plans in collaboration with the individuals receiving ACT services+ Monitoring, evaluating, and documenting the progress of the individuals toward treatment and recovery plan goals+ Providing educational resources as well as case management and advocacy as needed to support an ongoing recovery environmentRequired Experience+ At least 1 year of experience working with individuals with addiction issues+ Minimum 4+ years' experience in counseling field, preferably in Substance Abuse servicesRequired Education+ Bachelor's degree in psychology, social work or related field+ Certified Addictions Counselor in the state of PARequired Qualifications+ Knowledge of principles, procedures, and methods for behavioral health rehabilitation+ Strong desire to support individuals struggling with addiction issuesProgram SummaryAssertive Community Treatment (ACT) Team is a multi-disciplinary program that provides evidence-based, mobile, community mental health treatment to promote the recovery and community integration of individuals age 18 and up with serious mental illnesses, high rates of co-occurring substance use disorders, and high utilization of emergency/crisis services. ACT Team is a unique program that focuses on providing intensive outpatient treatment to individuals with serious mental illnesses so that they can maintain stability in the community."RHD employees and programs in the City of Philadelphia are required to follow the COVID vaccine mandate. Revised PDPH guidance allows for new hires to be partially vaccinated at the time of hire. This means they have received at least one shot in a two-dose series or a single dose in a one-dose series before beginning in-person shifts. The final dose must be received within 8 weeks of hire (start date). This currently does not include boosters.Resources for Human Development is an equal opportunity employer.
