Supervisor II is responsible for the daily operation of CareLink
programs as well as the implementation of agency mission, values
and goals. Individuals working in this capacity are accountable for
staff supervision, implementing quality rehabilitative programming
and ensuring the health and welfare of up to but no more than 12
service participants. Program Supervisors and Directors are part of
the Management Team and work closely with CareLink administration
to implement agency policies, procedures, and standards. This
position is responsible for supporting community integration. The
goal of this position is to promote the pursuit of independence for
all individuals participating in service. The essential functions
are established based upon the agency mission, values, and goals.
Educational Requirement: Master's degree or above from an
accredited college or university and 1 year work experience working
directly with persons with ID and/or autism, OR Bachelor's degree
from an accredited college or university and 2 years work
experience working directly with persons with ID and/or
autism.Qualifications: Minimum one year supervisory experience
required. CPRP certification preferred. Knowledge of psychotropic
medications, their uses and side effects required. Comprehensive
knowledge of methods of psychiatric rehabilitation required.
Ability to make sound judgements regarding the health and welfare
of service participants required. Ability to work as a leader and
provide training and supervision to staff required. Strong written
and verbal communication skills required. Motivation, interest and
ability to work empathetically with people who have disabilities
(intellectual/ developmental, mental illness, physical, emotional,
cognitive, etc.) required. Ability to work independently and
effectively handle multiple assignments required. Valid driver's
license required. Willingness and ability to drive agency vehicle.
Willingness and ability to handle on-call duties required.Essential
Functions: Plan the daily operation of the program. Report all
relevant problems and changes to the appropriate CareLink
administrative personnel. Assist staff in the planning,
coordination, implementation of the day, evening, and/or weekend
social and recreational activities for service participants.
Oversee referrals to the program. Coordinate and develop individual
service plans designed to improve the social and self-care skills
of service participants. Monitor participant medications. Oversee
service participants' special medication needs. Oversee menu
planning and food ordering on a weekly basis. Ensure coverage for
each shift. Work additional shifts when needed to ensure proper
coverage. Participate in the daily milieu of the program.
Demonstrate respect for service participants, families, staff and
agency principles. Assist staff in their duties when needed. Design
and implement services to meet individual needs. Transports
individuals as necessary/appropriate. Appropriately supervise all
staff, students and volunteers. Provide leadership, example,
instructions, suggestions, and discipline as appropriate. Monitor
individual service records on a continual basis to ensure that all
service documentation, plans, progress notes, outcome reports and
other materials are current. Provide a thorough and timely
orientation to each new employee in the program. Prepare staff
assignments. Participate in the development and training of
employees to improve their work quality and job knowledge. Assess,
teach and supervise staff on an on-going basis regarding their work
on individual service plans. Prepare and administer staff
introductory and annual performance evaluations accurately, fairly
and on time. Conduct staff meetings on a regular basis. Ensure that
program/site is in full compliance with all applicable regulations
and standards. Ensure that program site is well maintained. Monitor
program expenditures according to budget. Process all necessary
paperwork related to service participant program status, including
admission and discharge information. Monitor and sign-off on all
incident reports as needed. Ensure that all staff documentation is
completed accurately and on time. Handle liaison responsibilities
with appropriate entities. Participate in on-call rotation. Submit
all administrative paperwork on time as required. Respond
appropriately to emergency situations. Perform job within framework
of agency policies and procedures, program requirements and
professional standards. Contribute to Continuous Quality
Improvement meetings. Contribute to agency training schedule.
Receive and participate in supervision on an on-going basis.
Demonstrate the ability to take direction. Attend and participate
in agency events. Participate in achievement of agency goals,
objectives and mission through psychosocial rehabilitation values
and processes. Participate actively in quality improvement efforts,
including an emphasis upon individual and family satisfaction.
Prepare monthly billing reports. Maintain positive leadership role.
Maintain professional demeanor. Display motivation, interest and
initiative in working creatively with people with intellectual
disabilities and mental illness. Design and modify services to meet
the needs of all service participants. Demonstrate the ability to
make sound judgements regarding staff and participants. Demonstrate
knowledge of basic principles and methods of rehabilitation. Follow
expected dress code. Cooperate with other staff and work as part of
a team. Exhibit positive attitude toward staff, guests, position
and agency. Coordinate staff time-off, scheduling, hiring, and
time-reporting with Business and Human Resources Departments.
Monitor program expenses. Follow instructions accurately and
thoroughly. Complete assigned tasks within expected and designated
time frames. Perform job in a safe manner. Reports/addresses safety
