Job Description:

JOB PURPOSE:The PPC will be responsible for ensuring compliance with Adoption State and County regulations for children, youth and families for whom APM has received a Statewide Adoption and Permanency Network (SWAN) referral. The PPC will provide case management pertaining to permanency services to the children/youth, permanency resource parent(s) and foster parent(s) in accordance with service description and SWAN contract and program guidelines. DUTIES AND RESPONSIBILITIES:PPC must participate in bi-weekly one-hour supervision PPC maintains a maximum case load of 10 families/services PPC must complete 40 hours of training in the first year and 20 annually thereafter PPC must assess the physical condition of the resource homePPC must complete Post Permanency Services in accordance with SWAN guidelines and benchmarks. PPC must complete all required Post Permanency assessment tools and documentation PPC must complete time/cost log in the SWAN Portal in accordance with contract expectations. PPC must monitor contractor progress on assigned services to ensure timeliness and escalate any concerns to supervisor PPC must maintain up-to-date files, according to state and county requirements, and provide status updates on families/services PPC must work in conjunction with a multidisciplinary team to provide SWAN services PPC must complete mandated reports, documentation and benchmarks in the time frames specified within both the contract requirements PPC must notify supervisor of issues pertaining to matters related to assigned cases PPC must maintain documentation of supports, counseling and interventions provided to child/youth and resource parent PPC will maintain compliance with sharing information with external entities as needed for collaboration and incorporation into service plansPPC must assist with training and social work services to program families PPC must prepare for and attend regional meetings, statewide meetings and conferences, as well as service specific meetings and practice forums, court, etc. as requiredPPC must always use the approach See Something- Say Something- Do Something PPC must notify management, DHS and/or CUA if child or youth experiences circumstances or exhibits behaviors that require CM response or any safety concerns/incidents, taking immediate action to maintain the child or youth's safety and well-being PPC must effectively manage time to meet the agency and contract expectations PPC must maintain the confidentiality of clients PPC must follow guidelines of social work code of ethics Other duties as assigned to support the mission of APM/PraderaBENEFITS:Medical Insurance or $100/month medical insurance waiver reimbursement403b retirement plan: 4% APM contribution PLUS 3% APM matching contributionVisionDental20 PTO Days12 Paid HolidaysLife InsuranceLong and Short-term DisabilityAPM is an equal opportunity employer.PM21

QUALIFICATIONS:Minimum one year of experience working in the adoption system or other program with direct social service delivery (preferably in the Hispanic community).
