Job DescriptionDescription/Job SummaryThe RHD High Fidelity Wraparound Team is seeking a Family Support Partner (FSP). The FSP will work as part of a joint planning team to assist families and their youth with behavioral/emotional needs (age10-17). The candidate must have personal experience caring for a youth with behavioral health challenges and be knowledgeable of community resources and natural supports which provide youth and families with opportunities to succeed.What We Offer You:

      • Attend all mandatory county and state trainings
      • Must become complete training in the High Fidelity Wraparound Model
      • Must become credentialed as a Family Support Partner within 12 months of tra
      • Must become credentialed as a Family Support Partner within 12 months of training
      • Attend meetings as requested by the family, youth and/or other team members
      • Provide direct support (in a limited manner) within the family-driven plan
      • Support the implementation of the 10 Principles of High Fidelity Wraparound
      • Support the implementation of the 4 Phases of High Fidelity Wraparound
      • Work closely with team members to support positive outcomes for the family and youth
      • Assist with linkage to natural and community supports
      • Assist with crisis stabilization with neededRequired Experience
        • Personal lived experience with behavioral health challenges, the juvenile justice system, or child welfare
        • Possess life experiences that allow them to form healthy working relationships with the family and youth as well as team members
        • Knowledge of community and natural supports which provide youth and families with opportunities to succeedRequired EducationHigh School Diploma or GEDRequired Qualifications
          • Valid PA driver's license
          • Valid clearances including a criminal record, child abuse and FBI
          • Ability to develop, manage and maintain relationships with others
          • Ability to remain understanding, patient, culturally aware and respectful to the family, youth, and additional team members
          • Ability to role model effective behaviorsProgram SummaryRHD's High Fidelity Wraparound is a family-driven team planning process that follows a series of principles and phases as defined by the National Wraparound Initiative to help youth and their families realize their hopes and dreams. This is done through the identification of needs, building on strengths, incorporating the family's culture, building a support system and integrated planning."RHD employees and programs in the City of Philadelphia are required to follow the COVID vaccine mandate. The City of Philadelphia has issued an Emergency Regulation Governing the Control and Prevention of COVID-19 Mandating Vaccines for Healthcare Workers and In Higher Education, Healthcare, and Related Settings ("Vaccine Mandate Regulation"), which mandates vaccine for healthcare workers and in higher education, healthcare, and related settings. This mandate became Effective on August 16, 2021 and requires Healthcare Workers and certain workers at Healthcare Institutions to be fully vaccinated by October 15, 2021. Fully vaccinated means it has been two weeks since the final dose of vaccine. The regulation does allow individuals to apply to RHD for an exemption based on a sincerely held religious belief that would prohibit vaccination for COVID-19 or because of medical conditions that preclude administration of one of the available vaccines."
