CBH has an exciting role as a Spanish Bilingual Wraparound Facilitator working within our High Fidelity Wraparound team! -Position Overview: -The HFW Facilitator assists youth/families in learning to accomplish goals they have identified, by exploring the strengths and culture around each need they have and facilitating monthly interagency planning meetings to assist the youth/family in planning around and working towards meeting their needs. The HFW Facilitator will transfer their skills to the youth/family, by teaching the youth/family to think outside of the box and use natural and community supports to support them. -Essential Functions: -

• Facilitate monthly interagency planning meetings for a caseload of approximately 12 youth/families
• Attend all mandatory county and state trainings
• Adhere to the 10 Principles of High-Fidelity Wraparound
• Follow the 4 phases of High-Fidelity Wraparound and all activities associated with each phase
• Ensure that the Principles and steps of the High-Fidelity Wraparound process are delivered with the highest possible fidelity
• Create and maintain a team of system partners and natural supports for each adolescent and family
• Identify and link adolescents and families to community resources
• Write, revise and distribute individualized Wraparound Plans and Discovery documents
• Attend at least 3 hours of group or individual coaching/supervision per week
• Other non-essential duties as specified by the immediate supervisorPosition Requirements: -
  • Education: -Bachelor's degree (master's degree preferred) in a Human Services related field -
  • License/Certification: -
  • Valid PA Driver's License
  • HFW Facilitator Credential within 12 months of hire
  • Valid clearances including Criminal Record, Child Abuse and FBI must be received prior to start date
  • Relevant Work Experience: -
  • Experience working with residential treatment facilities
  • Understanding of adolescent and family resources and service systems within Philadelphia
  • Knowledge of child welfare system
  • Experience with crisis planning
  • Skills:
  • Ability to speak fluent Spanish and English

  • Ability to identify strengths of each adolescent and family and integrate these strengths throughout provision of process
