Position Overview: The population health pharmacist will serve in a consultative capacity, bringing pharmacy expertise to community-based care management (CBCM) and other clinical teams addressing youth and adult members with complex behavioral and physical needs. The populations of focus include but are not limited to Medicaid eligible pregnant, post-partum and inter-conception women, members with Severe Persistent Mental Illness and Children and youth with special health care needs. Essential Functions: * Assist in transitions of care through various levels--of care by developing and operationalizing Comprehensive Medication Review/Medication Reconciliation processes across clinical teams* Assess members' adherence to their medication regimen and address barriers to adherence - by implementing individual-level and population health level interventions using standard HEDIS measurements.* Provide education to members and their caregivers on their medications and the impact their medications have on treatment. * Provide education as warranted to other health care team members such as Certified Peer Specialists, Clinical Care Managers, Case Managers, Nurses, and prescribers.* Serve as primary liaison to PHMCOs and prescribers pertaining to medications and follow up (such as labs) to ensure optimal treatment and resolve of drug-related problems.* Support the implementation and evaluation of new clinical programs involving pharmacy personnel and analyzing programmatic reports to identify additional clinical opportunities requiring intervention.* Contributes to program improvement by using "continuous quality improvement" processes, being pro active in identifying improvement opportunities, and by contributing on-going consultation and collaboration internally within CBH and with external stakeholders. * Serve as preceptor to pharmacy and public health trainees. * Support the implementation of State-led policies and initiatives that enable CBH continually meets the Program Standards and Requirements as well as remain accredited by National committee for Quality Assurance. Position Requirements: * Education: Doctor of Pharmacy Degree (Pharm.D.)* License: Active--Pennsylvania Pharmacist License* At least 1 year of pharmacist experience in a clinical and/or public health setting. * Relevant Work Experience: CBH seeks to engage a clinical pharmacist with experience working in a healthcare setting serving individuals with behavioral health (mental health and substance abuse) disorders. The ideal candidate will have had experience interfacing with or working in a managed care or public health environment. * Knowledge of current research related to efficacy, adverse effects, and outcomes of administration of behavioral health medications, and strong knowledge of the prevalence of physical health disorders in individuals with behavioral health disorders across the life span.* Extra consideration will be given to a candidate with Post-Graduate Training or certification in Behavioral Health or Public Health.* Skills:* Technical: Must have experience and competency with Microsoft Office * Research: Introductory knowledge/experience with qualitative research and survey methods. Comfortable interfacing with data and analysis teams to participate in evaluation of programs.* Leadership and Initiative: Able to receive assignments in the form of high-level detailed goals to meet independently developed objectives, Self-motivated and thrives with autonomy. Willing to train, coach and mentor trainees. Takes accountability of own performance. * Communication: Excellent oral and written skills, Strong interpersonal skills with peers, multi-disciplinary professionals, and external stakeholders* Problem Solving and Quality: Strong organization and time management, with the ability to multi-task and juggle multiple priorities. Pays attention to detail, identifies, and solves complex issues. Thinks high-level when assessing and addressing problems.* Managing for Results: Comfortable to delegate, prioritize and manage the work of others. Balances competing priorities, scheduling issues and deadlines.
