Job DescriptionThe NewCourtland LIFE Physical Therapist functions as an integral part of the interdisciplinary care team, as this individual provides ongoing physical therapy support, clinical assessment, and intervention as needed to participants, in order to optimize the overall health status and quality of life for participants.The primary role of the Physical Therapist is to plan, organize, develop and direct Physical Therapy Services in accordance with current applicable federal, state and local standards, guidelines and regulations. The incumbent will work closely with the interdisciplinary care teams of both the LIFE Center and the NewCourtland Adult Day Center in the delivery of care and services according to each participant’s comprehensive assessment and plan of care.The LIFE Physical therapist will travel to participants homes, as needed, to provide care and follow ups.

  • Maintain compliance with policies, procedures, and regulations of both the Organization and of the Center; comply with policies addressing safe working conditions, and take initiative to resolve issues, etc., when unsafe conditions appear.
  • Implement infection prevention and control procedures so as to provide a safe, sanitary, and comfortable environment and to prevent the transmission of disease and infection; practice in a manner consistent with infection control procedures.
  • Actively participate in the Quality Improvement/Assurance Committee with other members of the interdisciplinary care team.
  • Complete regular and consistent documentation re: participant progress towards identified goals and outstanding issues/concerns (i.e., monthly and as needed progress notes, and initial and bi-annual care plans for all participants, as related to physical therapy goals, per regulatory requirements).
  • Complete required documentation, intervention, and follow-up of special circumstances including accident/incident/grievance reports, and maintain compliance with state and federal laws and regulations.
  • Identify functional problems and establish goals for the plan of care, so as to improve and/or maintain each participant’s functional status; periodically re-evaluate and upgrade participant therapy programs to maximize functional improvement.
  • Administer individualized exercise program utilizing specialized therapeutic exercise techniques and equipment.
  • Plan and administer prescribed physical therapy treatment programs for participants to restore function, relieve pain, and prevent disability following disease, injury, or loss of a body part.
  • Collaborate routinely with Therapeutic Program Coordinator to ensure that Physical Therapy goals are carried out within the scope of daily Program activities.
  • Maintain ongoing communication with the interdisciplinary care teams of both the Adult Day Center and the LIFE Center regarding participant issues, concerns, and needs related to physical therapy assessment and care plan progress.
  • Maintain ongoing communication with participants, and if applicable, their families/caregivers regarding participant issues, concerns, and needs related to physical therapy assessment and care plan progress.
  • Confer with participant, participant’s family member/caregiver, and the interdisciplinary care team to obtain information so as to promote coordinated and efficient care for each participant.Must be licensed and/or eligible for licensure in the state of Pennsylvania.At least two years of professional physical therapy experience in a long-term care facility, outpatient and/or community-based care setting, working directly with the elderly and/or disabled populations. Must be proficient with computer applications and have the ability to learn and utilize specialized Adult Day Center programs.EOECompany DescriptionSeniors. Services. Solutions.
