8:30 am to 5:00 pm On-call Schedule: Monday-Friday from 5 pm to
8:30 am | Weekly rotation between team units| No weekends

adoption, child welfare, mental & behavioral health, community and
economic development, housing, community school services, and
violence intervention services to the North Philadelphia
communities. The Community Umbrella Agencies (CUA) are
community-based agencies established to provide case management
services and ensure the permanency, wellbeing, and safety of the
children, youth, and their families. APM CUA is looking for
compassionate and committed social workers who want to create
long-lasting impacts in the lives of children and families. APM CUA
2 services the Lower Eastern North region and CUA 5 services the
Logan/Olney region. JOB SUMMARY The CUA Case Manager plays a
critical role in supporting families involved in the child welfare
system. This position is responsible for managing a caseload of
children and families, conducting regular home visits, developing
service plans, and coordinating with various stakeholders to ensure
the safety and well-being of children. The Case Manager serves as
the primary point of contact for families and is instrumental in
helping them achieve reunification, kinship care, or other
permanency goals. Key Responsibilities Case Management & Planning
Maintain a caseload in accordance with DHS and agency standards.
Conduct comprehensive assessments of family strengths, needs, and
risks. Develop and implement Family Service Plans (FSPs) in
collaboration with families and service providers. Monitor progress
toward goals and adjust plans as needed. Family Engagement &
Support Build trusting relationships with children and families
through regular home visits and consistent communication. Provide
crisis intervention, advocacy, and referrals to community
resources. Support families in navigating systems such as
education, healthcare, behavioral health, and housing.
Collaboration & Coordination Work closely with DHS, legal
representatives, schools, medical professionals, and behavioral
health providers. Attend and participate in court hearings, Family
Team Conferences, and other multidisciplinary meetings. Ensure
timely and accurate documentation in the electronic case management
system (ECMS). Compliance & Reporting Adhere to all DHS, state, and
federal regulations regarding child welfare services. Submit
required documentation, reports, and case notes within established
timeframes. Participate in regular supervision, training, and

a Bachelor’s Degree in Social Work or related field including but
not limited to sociology, psychology, counseling, criminal justice,
education, divinity, or public health administration A minimum of 1
year of experience in human services preferred Case Management
Skills Conducting assessments Developing and implementing service
plans Monitoring progress and adjusting plans as needed Knowledge
of Child Welfare Systems Familiarity with DHS policies and
procedures Understanding of trauma-informed care and permanency
planning Crisis Intervention Ability to respond to emergencies and
de-escalate situations Making appropriate referrals for mental
health, substance abuse, or domestic violence Empathy and Cultural
Competence Working effectively with families from diverse
backgrounds Building trust and rapport with children and caregivers
Collaboration Coordinating with schools, courts, healthcare
providers, and other service agencies Participating in team
meetings and family group conferences Strong Verbal and Written
Communication Writing clear, concise case notes and reports
Communicating effectively with families, supervisors, and
stakeholders Time Management Managing a high caseload while meeting
deadlines and documentation requirements Attention to Detail
Ensuring compliance with legal and procedural standards Tech
Proficiency Using case management software and digital tools for
documentation and communication Clearances Ability to obtain FBI
fingerprints, PA criminal record clearance, PA child abuse
clearance, medical exam and TB test Valid driver's license with
