excellence in healthcare. SUMMARY OF JOB: Delivers quality hospice
and palliative nursing care to patients in the community, nursing
home, or inpatient hospice setting. Develops, implements, and
coordinates the interdisciplinary plan of care for each patient and
family. Participates in professional development activities that
further personal and program goals related to end of life care.

internal and external customers. RECRUITMENT REQUIREMENTS: Graduate
from an accredited school of nursing. BSN preferred Minimum 1 year
med/surg, nursing home, or rehab or 2 years' experience as a
professional nurse. 1 year home care or hospice experience
preferred. Able to meet weekend and holiday requirements. LICENSE
AND REGULATORY REQUIREMENTS: Current licensure in State of
practice, Valid Driver's License, Current CPR Certification EQUAL
