Shift: Day Shift Hours per Shift: 8:00 AM - 4:30 PM Experience: 1
years of recent Home Health experience required. Case management
experience required. License: Active License (verification must be
less than 30 days old). Certifications: BLS (AHA) Must-Have: -
Clear driving record - Active driver's license - Current auto
insurance - Proof of driving credentials within 48 hours of offer
acceptance - IV and venipuncture experience (highly preferred) -
Homecare Home Base EMR experience (required) - Previous travel
experience (highly preferred) Description: A Home Care Visiting RN
provides safe, holistic care to a population with multiple
co-morbidities and advanced chronic illnesses. The RN educates
regarding self-management and provides support to meet the needs of
the patient, family, and community. Mileage is reimbursed. The
territory includes specific designated zip codes. The RN will see 6
patients per day and is expected to complete visits, start of care,
recertifications, and discharges.
