Job Title: Registered Nurse Assessment CoordinatorWesley Enhanced Living is seeking a highly skilled Registered Nurse Assessment Coordinator to join our team. As a key member of our nursing leadership team, you will be responsible for providing initial assessments and periodic re-evaluations of residents admitted to our Skilled Nursing facility.Key Responsibilities:

• Ensure accuracy of Multiple Data Set (MDS) coding to maximize company reimbursement.
• Conduct Interdisciplinary Team Care Conferences and implement and update resident Plans of Care.
• Provide nursing education and training to staff members.
• Manage clinical and financial record keeping and dissemination of records to payers.
• Collaborate with social workers to develop care plans.Requirements:
  • Minimum 2 years of related experience.
  • Graduate of an accredited School of Nursing.
  • Current license with the State of Pennsylvania for Registered Nurse.
