The Field Clinical Coordinator RN will be the primary care manager for members with High Risk Pregnancies and infants requiring a NICU level of care. -Care coordination activities will focus on supporting member's medical, behavioral, and socioeconomic needs to promote appropriate utilization of services and improved quality of care.
Schedule: Monday through Friday 8:00 to 5:00 pm Central Time Zone
Location: This is a field-based position with a home-based office. You will work from home when not in the field.
If you are located in Louisiana, you will have the flexibility to work remotely* as you take on some tough challenges.
Primary Responsibilities:


  • Engage members face-to-face and/or telephonically to complete a comprehensive needs assessment, including assessment of medical, behavioral, functional, cultural, and socioeconomic needs
  • Develop and implement person centered care plans to address needs including management of chronic health conditions, health promotion and wellness, social determinants of health, medication management and member safety in alignment with evidence-based guidelines
  • Partner and collaborate with internal care team, providers, and community resources/partners to implement care plan
  • Provide education and coaching to support member self-management of care needs and lifestyle changes to promote health
  • Support proactive discharge planning and manage/coordinate Care Transition
  • Advocate for members and families as needed to ensure the member's needs and choices are fully represented and supported by the health care team
  • Coordinating care for high-risk maternity and NICU members

      • Bachelor's degree (or higher) in Nursing (RN) or 2+ years of equivalent nursing experience
      • Unrestricted RN license in the State of Louisiana
      • Intermediate level of computer proficiency (including Microsoft Outlook) and ability to use multiple web applications and enter/retrieve data in electronic clinical records
      • Ability to travel locally approximately 50% of the time and up to 100 miles round trip with reliable transportation, that will enable you to travel to client and/or patient sites within a designated area, with a current and non-restricted state of Louisiana driver's license and state-required insurance
      • Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements.         Preferred Qualifications:

        • Certified Case Manager (CCM)
        • 2+ years of Maternal/Child Clinical experience or 1+ years of experience as a Maternity Case Manager for an insurance compan
        • Psychiatric or Behavioral Health experience
        • Medicaid experience
        • Case Management experience
        • Experience working with chronic illness and complex care needs population
        • Experience in utilization review, concurrent review and / or risk management
        • Background in managed care
        • Bilingual in English and Spanish
        • Proven ability to communicate complex or technical information in a manner that others can understand, as well as ability to understand and interpret complex information from others
