ResponsibilitiesSchedule: 4 -10hr shifts or 5 - 8hr shifts
(Monday-Friday)Responsible for coordinating care for a group of
patients for a designated period to achieve clinical, financial,
and organizational outcomes. Provides direct patient care and
education based on the nursing process; mentors and manages others
to promote quality and excellence of care, supports physicians with
clinical and laboratory procedures. Works collaboratively with, and
provides leadership for, a multidisciplinary health care team to
optimize team performance and patient outcomes. Promotes
improvement in the practice, processes, and systems to benefit
patient care and achieve practice and organizational goals.
Incorporates and operationalizes goals and objectives of department
at the site of patient care. Decision making within the scope of
nursing care and responsibilities.\tManage Post Discharge office
follow-up visits, Imaging, and Endoscopy Procedure follow-up needs.
Facilitate scheduling of post discharge visits with MD or
APP.\tPost Discharge care coordination, CT, MRI - auths, follow-up
with PCP, etc...\tProvide support to clinical staff and physicians
to facilitate Direct Inpatient Admissions. \tGI Biologic and Iron
Infusion Coordination - to include placement of orders, working
with Infusion Centers and TJUH Auth team to facilitate scheduling
and authorizations. Provide patient teaching for injectable
medications and biologics. Work with Pharma to ensure that patients
have sample biologic drug during pending authorization
process.\tTube feeding coordination, work with Home Care Nursing
Agencies, Medical Equipment Suppliers, Dieticians and Physicians to
ensure patients are receiving appropriate nutrition
therapy.\tCoordination of Insurance Peer to Peer denials, Appeals
for Procedures, Medication, Imaging services\tManagement of
workflow for Cardiac and other Medical clearances required for
Endoscopy Procedures.\tAssesses and documents patient's weight,
height, and vital signs.\tUpdates patient's record at each visit
with medications, immunizations, medical or surgical
procedures.\tAssesses patient's health literacy, providing patient
support with intake questionnaires and completion of requisite
medical record information, e.g. obtaining and maintaining current
and accurate histories.\tPrepares patients for
examination.\tAssists with procedures, specimen collection, and
testing as needed.\tAdministers injections as directed by the
physician.\tReviews prescription renewal requests and responds to
patient prescription questions. Telephones in prescription renewals
or orders as directed or requested by the physician.\tProvides
patient education\tKnowledge of all medical procedures performed in
the office.\tEducate patients concerning testing procedures and
follow-up.\tManages intake of all patient telephone calls into the
department for prescription requests or inquiries and medical
concerns.\tRetrieves patient phone messages, manages and resolves
phone messages for physicians, if possible, and documents the
patient's call and the department's response in patient chart.
Triage patient telephone calls to identify issues that require
physician attention.\tPerforms pre-certification of prescription
medications.\tCommunicates with pharmacists regarding prescriptions
and renewals as directed by physicians\tCommunicates with patients,
families and other health care providers\tCalls patient with test
results, as directed by the physicians\tFacilitates the
department's completion of patients' forms from such outside
sources as disability, workers compensation, schools and
DOT.\tResponsible for maintaining schedule of lumbar puncture
procedures, ensuring that the appropriate supplies and equipment
are available for the procedure, and coordinating
appointments.\tAct as a liaison between patient, families, MD,
social work, and neurology fellows/residents as needed.\tDevelops
and updates clinical policies within the outpatient
setting\tInteracts with co-workers, visitors, and other staff
consistent with the core values of the
University.QualificationsEducation: ASN Nursing Diploma Required.
BSN preferred.Licenses: Registered Nurse in the Commonwealth of
PennsylvaniaExperience: 3 years of experience as a RNConditions of
