Disability!JOB SUMMARY:To promote the mission of PATH to help
individuals achieve a more independent and fulfilling life by being
an effective team member. The Behavioral Health / Intellectual
Disability (BH/ID) Specialized Team provides direct care including
support, structure, assistance, skill training and therapeutic
interventions, for service participants, in collaboration with
BH/ID residential staff. Services are provided in the home and
community.QUALIFICATIONS:Bachelor's degree required2 years of
mental health experience working with individuals in mental health
and/or co-occurring settings2 years of experience in human
services, paid or unpaid, preferably with adults with psychiatric
and/or intellectual disabilitiesActive Driver's license
requiredCOVID-19 vaccination, or willingness to receive vaccine
requiredSPECIFIC DUTIES:Provide supervision of residents at all
timesMaintain a supportive, therapeutic relationship with each
service participantBe aware of each service participant's
individual service plan, history, medications, current needs and
any special accommodations required by the participantAct as a role
model at all times, maintaining appropriate boundaries and
demonstrating effective communication skillsMaintain clear and
consistent expectations of each service participant while
maintaining a safe, supportive environmentImplement, on a
consistent basis, direct service interventions as defined in the
individual's service plansOversee, teach skills and actively assist
in the completion of household chores ensuring the home is neat and
clean at all timesAssist service participants in developing and
practicing skills defined on their individual service plan. This
may include accompanying residents to appointments, social
activities and/or job opportunities, and other activities of daily
living.Maintain open and ongoing collaboration and communication
with all team members including the clinician, psychiatrist, nurse
and peer specialistMaintain ongoing communication with each service
participant's family/significant others utilizing diplomacy and
tact and creating a welcoming environmentEnsure any family concerns
are shared immediately with a supervisor Participate in household
shopping, as assigned Coordinate, facilitate and engage in regular
social activities with the service participantsTransport and
accompany residents to appointments and activities, as
assignedProvide program coverage/shift coverage as assignedOther
duties as assignedPHYSICAL DEMANDS:Must be able to assist in crisis
management interventions, administer CPR/First Aid, and assist
