We are looking for a Chronic Care Physician to join a premier home-based health system in the Philadelphia, PA area:

Permanent, full-time position
Internal or Family Medicine Physician with Geriatric background preferred
Join a team that places an emphasis on quality of care with excellent support staff
Average of 5-6 patients per day
Excellent compensation and benefits plan
Minimum (2) years experience required
Very minimal call (Telehealth)
