PHMC requires that all employees are fully vaccinated with the Covid-19 vaccine before the first day of employment.West Haven is a 16 bed long-term structured residence for adults with serious mental illness. The Nurse Coordinator provides professional nursing services including case management, health education, counseling, supportive and restorative care to all residents and provides coordination and direct supervision of all nursing staff. The Nurse Coordinator is a member of the West Haven Leadership Team and is responsible for promoting the welfare of the program.Responsibilities:

  • Provide and coordinate supportive and restorative care for all West Haven residents including management of physical health conditions including diabetes, COPD, and infections to promote optimal resident health
  • Provide and coordinate nursing case management, health education, and counseling for all WestHaven residents to ensure client's medical, psychiatric, and psychological needs are met
  • Coordinate the schedule to ensure 24-hour nursing coverage
  • Provide supervision for all nursing staff
  • Conduct Annual Performance Evaluations with all nursing staff
  • Attend treatment team meetings, leadership meetings, round up meetings and state and city review meetings to facilitate information exchange between nursing staff and other team members to promote residents' recoveries
  • Complete nursing health assessments on all residents within 24hrs of admission and annually thereafter; ensure a safety assessment is completed during this time frame
  • Review all lab results and ensure appropriate follow-up is performed
  • Coordinate all nursing related billing issues including laboratory and pharmacy billing and work with the finance department to ensure prompt payment
  • Conduct monthly and quarterly chart reviews and audits to ensure quality indicators are met
  • Coordinate outside resources providing health education to residents
  • Act as the primary liaison to contract pharmacy and laboratory vendors
  • Manage Electronic Medication Administration Record system across all shifts
  • Perform involuntary commitments as needed and ensure all nursing staff is appropriately trained to do so
  • Maintain Standard Precautions among staff and residents including coordinating all infection control activities and report findings monthly
  • Coordinate resident's physical examinations within 24 hours of admission
  • Oversee resident medical services in areas such as scheduling of medical appointments, prescriptions, dental care and emergency room visits
  • Coordinate all resident prescriptions with pharmacy ensuring that all medical, demographic and insurance information is submitted prior to sending prescriptions
  • Utilize the resident's primary care physician to coordinate medical referrals needed for further assessment or management of episodic and/or chronic health problems
  • Follow through with resident and provider to determine the outcome of medical visits
  • Maintain communication between WestHaven psychiatrist and the resident's Primary Care Physician for medical issues requiring ongoing treatment
  • Coordinate annual psychiatric evaluations for all residents with consulting psychiatrist
  • Review all lab results and notify Consulting Psychiatrist regarding abnormal results and follow-up as instructed, document actions taken
  • Complete shift review of all medication, and treatment orders for the day, initial, and validate correct transcription of orders
  • Coordinate medication and medical supply ordering across all shifts
  • Oversee the administration of resident immunizations and self-administration of medications under the supervision of the consulting psychiatrist
  • Provide in-service training for staff and residents (specifics to be identified)
  • Provide individualized health education to all residents
  • Adhere to all WestHaven Policies and Procedures
  • Adhere to regulatory guidelines as outlined by CBH, OMHSAS and other regulators
  • Work with Program Director to ensure Reportable Incidents are entered into HCSIS within 24 hours and are properly released to all other appropriate agencies. Ensure follow-up investigations are completed accurately for medical matters
  • Adhere to all medical documentation requirements
  • Submit monthly reports to the Program Director detailing nursing activitiesSkills:
    • Knowledge of medical and biopsychosocial issues relevant to chronic and severe mental illnesses in the adult population
    • Ability to work with and educate non-medical staff
    • Experience, knowledge, and demonstrable skills in De-escalation Management
    • Computer literate
    • Current CPR certification
    • A valid Pennsylvania Driver's License
    • Good verbal and writing skills
    • Knowledge and understanding of regulatory entities standards of care and ability to implement policies and proceduresExperience:
      • A minimum of 4 years working with chronically mentally ill adults, LTSR experience preferred
      • Experience working with diverse populations and knowledge of diverse cultural practices, beliefs, and culturally congruent care
      • Experience supervising professional and non-professional personnelEducation Requirement:
        • RN with an Associate or BSN degree with public health or community nursing experienceLicensure:
          • RN licensed in PennsylvaniaSalary:
            • Commensurate with experience
            • Grade 21
