Works collaboratively with the patient to foster self-management and compliance to their clinical plan of care. Works collaboratively with physician, hospital and community resources to support the patient's clinical plan of care.Transition Navigators will work remote and travel to personal care homes.RequirementsEducation Bachelors Degree : in Nursing (Required)Masters Degree : in Nursing (Preferred)Experience 3 Years experience in disease or case management services with focus on telephonic management, medical reconciliation and ambulatory care coordination (Required)General Experience with clinical, hospital-based information systems (Required)License/Certifications RN-LIC - PA Registered Nurse License (Required)Quality Nurses Job ID #m228369.
