Job Description:

SUMMARY OF JOB: A Triage RN receives and prioritizes all incoming calls in order to continue delivery of safe, holistic care to a population with multiple co-morbidities and advanced chronic illnesses with a goal of preventing readmission or hospital admission when appropriate. Provides external telephonic phone visits to assess patient status in an effort to prevent adverse health event or hospital admission. The Triage RN implements the nursing process during phone encounters to assess the patient and caregiver situations and provide appropriate support including education of the disease process to the patient and family. In order to effectively collaborate with scheduling, management and others, Triage team working during core business hours are facility based positions. Night Triage team is home based.

A primary way staff connect to our mission is by collaborating with all team members in promoting an environment that provides quality, compassionate, holistic, patient-centered care and supports all internal and external customersRECRUITMENT REQUIREMENTS: Minimum of 2-3 year med-surg experience required; 1 year of home care/hospice experience required; BSN preferred; ER/Triage experience a plus; able to work weekend, holidays; able to demonstrate professional competencies in a home care setting; able to efficiently utilize technology (cell phone, computer).LICENSE AND REGULATORY REQUIREMENTS: Dually RN Licensed in PA and NJ Current CPR CertificationEOE
