Job Description:
NEW *$2500 SIGN ON BONUS* NEW SUMMARY OF JOB: A Home Care
Admission RN provides safe, holistic care to a population with
multiple co-morbidities and advanced chronic illnesses. The RN
performs an initial assessment of patient's needs, admits to the
Agency based on Medicare and Agency policy and initiates the care
planning process. The RN may also educate regarding self management
and provide support to meet the needs of the patient, family, and
community. RECRUITMENT REQUIREMENTS: Minimum of 1 year med-surg
experience required; 1 year of home care experience preferred; BSN
preferred; able to work weekend, holiday, and on-call hours; IV and
venipuncture experience preferred; able to demonstrate professional
competencies in a home care setting; able to efficiently utilize
technology (cell phone, computer); able to travel in service area.
LICENSE AND REGULATORY REQUIREMENTS: Current State RN License,
Valid Driver's License, Current CPR Certification EOE
