Responsibilities

* To coordinate services, programs, and personnel necessary to facilitate the patient through the continuum of care.
* Facilitates and insures the achievement of goals set by the Case Management/Performance Improvement programs such as quality patient care, timely and appropriate resource use and minimal financial risk.
* Collaborates closely with the health Care Team to provide a coordinated experience to patients and families.

Qualifications

* Basic educational qualification for professional affiliation required. Bachelors degree in a Nursing or equivalent training and proven experience with applied practice in an appropriate healthcare discipline.
* Current RN licensure in the state of Pennsylvania
* Strong, 2 years, acute care background required. Case Management / Utilization review experience preferred
