Time position. Our hours are Mondays through Fridays from 8:30 am
to 5 pm. This position has an on-call component from Mondays
through Thursdays rotating between teams every 4-5 weeks.Why Work

Worker, part of your job is:Reviews case record for clients and
family's profile, socio-economic history, previous treatment and
service experiences, and/or special problems and family history,
financial and social problems, individual perspectives and
perceptions. Develops service plan to provide a variety of social
services referrals and to define goals and objectives; determines
need for social, behavioral, medical and/or psychological services;
provides individual or family counseling as needed; makes referrals
to a variety of support agencies; monitors individual and family
progress, cooperation, and acceptance of services. Plans for the
appropriate placement of individuals in a kinship or family foster
home, care program, facility or institution; interviews prospective
applicants, clients and providers; orients participants in
program's goals and objectives, roles and responsibilities of
various parties, and legal ramifications; arranges for any medical
or psychiatric treatment prior to placement. Monitors and evaluates
activities of agencies contracted to provide a variety of social
services; visits and inspects direct placement to evaluate
progress, and/or problems of client; prepares and discusses written
evaluations of agency; finds alternative placement for client when
warranted; recommends services provided to client by agency be
terminated. Initiates court action when appropriate and prepare
necessary documents. Attends periodic staff and personal
conferencesConfers with superior on difficult problemsPrepares
reports and correspondenceMaintain of all assigned cases. Obtains a
minimum of twenty (20) hours of training per fiscal year. A minimum
1 hour supervision bi - weekly, group and other staff meetings as
required.Participate in case conferences, school conferences, all
teaming conferences and court appearances with the family as
requested as well as any inter disciplinary meetings deemed
necessary to support movement of the case.Provision of family
prevention and intervention services to assess the potential for a
minimum standard of parenting, as well as rehabilitating and
stabilizing family relationshipsProvision of on-call services in
cases of emergency and on a rotational basis as required by APM CUA
protocolPerforms related work as required. IMPORTANT: APM requires
all new hires to obtain the following clearances: FBI, Child Abuse
Clearance, Criminal Background, and Medical/TB Test. APM is an

From You? Minimum of a Bachelor's Degree in a Social Work or a
related field, plus 2 years experience in Human Services
(preferably Child Welfare). Some related fields are, social work,
sociology, psychology, theology, education, criminal justice and
public health administration.Excellent verbal and written
communication skillsStrong organizational skillsStrong clinical
writing skillsSound judgment, critical thinking, and
problem-solving skills are essential.Valid Driver's
licenseFamiliarity in computers including word processing programs
and internetKnowledge of social work values and/or child welfare
services preferredBi-lingual in English and Spanish (preferably)
