Work primarily with adults with serious mental illness or emotional disorders, assisting individuals to make full use of Behavioral Health Services and natural community supports to achieve a more independent and fulfilling life. Services provided will include developing a partnership with the individual to assess needs, obtain services, and resolve crises. QUALIFICATIONS:

  • Bachelor's Degree in Human Services field from an accredited college/university
  • One to two years experience in the behavioral health field
  • Use of own personal vehicle and valid driver's license required
  • Experience working with children and adolescents preferred
  • Good interpersonal and organizational skills
    • Ability to use electronic health record required
    • COVID-19 vaccination, or willingness to receive vaccine required SPECIFIC DUTIES:
      • Develop partnerships with assigned individuals to assess needs, obtain services and resolve crises
      • Assist individuals with identifying goals and defining the steps needed to achieve their goals
      • Complete all assessment and planning documentation including Adult Psychosocial Assessment; Strengths Assessment; Treatment Plans and other required documentation
      • Ensure all required documentation is completed in a timely, comprehensive, and legible manner and is in compliance with PATH policies and procedures
      • Assist in linking individuals with their chosen services and resources in the community such as needed psychiatric and medical treatment, residential services, financial entitlements, social/recreational activities, and general social services
      • Monitor those supports and services for appropriateness and effectiveness
      • Provide informal support and network building to help individuals increase their support system outside of the Behavioral Health System
      • Actively participate in discharge planning
      • Complete all paperwork in a timely, comprehensive, legible manner and in accordance with PATH policies and procedures
      • Attend all meetings for individuals on caseload as well as other assigned meetings
      • Respond to all emergencies and crises for individuals on caseload and mobilize available resources to assist the individual in crisis
