Nationwide Healthcare Services - Admissions Liaison/Marketing Director
Nationwide Healthcare Services is seeking an Admissions Liaison/Marketing Director for the Philadelphia and surrounding areas.
Essential Duties include but are not limited to:
- - - - - - - - Assessing referred patients to determine appropriate placement based on facility's admission criteria.
- - - - - - - - Coordinates communications between the hospital discharge planners, case managers, social workers, medical staff and caregivers regarding discharge from hospital and admission to skilled nursing facility.
- - - - - - - - Responsible for creating and fostering positive relationships with all community referral sources and potential admission into one of Nationwide Healthcare Centers.
- - - - - - - - Manages referral sources including but are not limited to hospitals, physicians, nurse practitioners, and home care agencies.
- - - - - - - - Works in conjunction with the facility's admissions representative to increase our referral base and help to maintain and grow census.

Qualifications for the Admissions/Liaison/Marketing Director:

-.3 -5 years of Long Term Care experience in the Admissions department
