Responsibilities of Social Worker:

Actively involved in the pre-admission and admissions processes
Listens to and acts compassionately with residents and their family members while providing individual and group help when needed
Maintaining and/or improving resident's ability to control everyday physical needs and mental/psychosocial needs
Initiating appropriate interventions for residents experiencing mental and behavioral health issues
Identifying individual social and emotional needs
Develop and maintains individualized care plans
Participate in the planning of the resident's admission, community re-entry, or transfer to another facility
Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
Work with the finance team to ensure the resident is educated and the proper documents are in place for the resident during his or her stay
Work with the resident on discharge and get necessary DME or home care in place so the resident is safe when he or she returns to the community

Qualifications for Social Worker:

Bachelor's degree in a related field (BSW or MSW preferred)
Minimum of one (1) year of experience in the Social Work field
Experience in Minimum Dates set 3.0(MDS) Assessments and use of EMR for charting
Strong ability to manage, multi-task, and communicate effectively
Excellent work ethic, intense drive, and initiative for quality service
Long-term care and/or sub-acute experience preferred
