JOB SUMMARY:To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. To work as part of the interdisciplinary team to provide crisis intervention, support, safety plans and linkages to children, youth and families in Philadelphia. The UCC program operates 5 days/week Monday through Friday, seeing youth typically between 11 a.m. until 7:00 p.m. PHYSICAL DEMANDS:Minimal in natureQUALIFICATIONS:Bachelor's Degree in the Human Services field from an accredited college/university; One to two years experience in the behavioral health field. Experience working with children and adolescents preferred; Basic, ability to use an Electronic Health Record; Good interpersonal and organizational skillsSPECIFIC DUTIES: To provide crisis management that is coordinated and integrated with the Interdisciplinary Treatment Team of the UCC.To provide telephone crisis and triage coverage during hours of operation in order to arrange for assessment, make referrals to appropriate services, and provide education and support of ongoing care.During enrollment time, collaborate with CBH for eligibility verification, current and history of treatment services and provide this information to nurse and crisis clinician. Obtain language/translation services, as needed, to work effectively with youth and families.Assist crisis clinicians in provision of crisis intervention, management, parent education and treatment interventions.Utilize approved evidence-based tools and practices including SAMHSA crisis guidelines, Crisis Resolution model and Motivational Interviewing.Assist in developing a plan with input from the treatment team, for each individual and /or family member assessed. The plan will include ongoing follow-up, as indicated, beginning the day after the crisis assessment to ensure that the process of linkage and stabilization has been initiated. Assist in mobilizing resources and supports, to include obtaining and maintaining culturally appropriate basic living needs including but not limited to housing, food, medical care, recreation, and education.Connect with any current treatment providers of consumer to inform of encounter and collaborate regarding needs and resources.To provide follow-up care and/or transfer to other levels of care from the moment that an assessment is completed, and the plan has been developed.
