A healthcare organization is hiring an Service Coordinator/Intake Representative for its NHTD (Nursing Home Transition and Diversion) program. In this role, you will guide individuals through the enrollment process, collect required documents, and coordinate with healthcare providers. While prior experience is not required, an Associate’s or Bachelor’s degree is essential. The ideal candidate will be organized, a strong communicator, and passionate about helping others. Spanish proficiency is a plus. Responsibilities Include: Enrollment Support : Guide individuals through the enrollment process for the NHTD program. Collect and organize necessary documents for program participation. Provider Coordination : Coordinate with healthcare providers to ensure a smooth enrollment process and timely documentation. Communicate with program participants and providers to clarify any necessary details. Data Management & Reporting : Maintain accurate records of participant documentation and progress. Assist with reporting and ensure all necessary paperwork is up-to-date and compliant. Ideal Qualifications: Associate’s or Bachelor’s degree required. Strong organizational skills and attention to detail. Excellent communication skills, both verbal and written. Ability to work independently and as part of a team. Spanish fluency is a plus.
